Always wondered about this texture

I've always wondered what this thing is supposed to be and what it does.

Maybe that skin is it's blanket, and all the gunfire woke it up?

it looks like photograph (the shading does not look like most doom walls or even the rest of the texture) of someones skin with bruise or scab on it, I always thought the red things were blood stained nails or screws holding it into the wall, but just guessing :)

Here is an article with some background information. In short, that texture is most likely a digitized image of a bloody scab on Kevin Cloud's knee.

The top edge of the skin looks like a lip, and it's smiling.

I see the red things as eyes, the purple part as a mouth and and a tongue coming out of it.
