It's from the Doomguy's haircut thread -that one used to be choke-full of "cursed images" anyway... more specifically, that "floating Doomguy's head" sprite pack is from this ZDoom mod.

 

Apparently it had an earlier version which somehow made it look even dumber:

I dunno, especially the full lateral view from the earlier version, with the prominent chin and nose, reminds me a lot of Karagiozis.

I've never watched Star Trek, so this WAD actually has nothing to do with that puzzle.

 

This is just a crap Doom WAD (and .DEH) made by me when I was 12 years old.

 

The metal texture is a custom texture I made for the WAD that was meant to resemble the metal scaffolding commonly seen on many different buildings, especially in my homeland New York City.
