[Historical] D!ZONE level contest?

Been meaning to ask about this one for a while - I have a box copy of D!ZONE Collector's Edition that contains this interesting flyer:

Didn't find any mention of this on the Doom Wiki, or on a very cursory web search, so, anyone know anything more about this?  Did it happen?  Who won if so?  Does it relate to WizardWorks becoming the publisher for Perdition's Gate and Hell to Pay in any way?

 

(Tangentially, I'm also curious what happened with Crack Dot Com's Abuse level design contest, but that's a bit off topic to Doom stuff.)

I wonder what that PWad level is. Judging from the sky texture, it's probably for DOOM II.
