As punishment for their failure, they get sent to the exact same place in the exact same level to be killed again by the next Doomguy that visits it.

No idea about current canon with Doom Eternal, but in classic Doom, dead demons would be either brought back by the Archvile, or get back together into a makeshift piece of shit, also known as the Revenant.

Which begs the question: how do a dead imp and a dead cyberdemon both end up looking like the remains of a dashing 7ft tall human?

The skeletons are mass-produced, they just cram just enough of the corpse meat to hold a demon soul in the armor piece.

Nowhere, at least in the classic games. All of them, no matter how many, stay where you left them, which is a cool thing I think was sorely lacking in the more recent games.
