There's actually a story behind this: Apparently, a WW2 veteran mailed id over it and objected to its presence, so Romero decided to remove it.

Let me tell you something: If your perception of the Red Cross has been irreparably damaged by its use in video games, or if you feel confused about the purpose of the Red Cross after seeing its symbol so disgracefully abused by a James Bond film, you are suffering from brain damage so severe that the Red Cross cannot help you anyway. You need cerebral maggot repellant to kill the worms in your brain, not the Geneva Convention. If it is so important on a basic moral level to never, ever use the 🏥 sign, which is conveniently a stock emoji on my Android phone that I never knew I had, when are we going to start discouraging its use in community content and start forcing people to use the Innocent Pill symbol instead? Actually, I'd recommend not doing that due to the fact that pills are meant to help people, and if someone sees a pillbox used in a violent game alongside depictions of demons and other fictional things that cavemen haven't yet learned to separate from reality, a totally innocent old man might see it and decide to stop taking their blood pressure medicine. While I understand why developers no longer use the symbol since it absolves them from controversy, I couldn't give a shit less, in terms of morality, if it is used in entertainment products.

The H and the heart were def one of the "finalists" (I had like 9 different variations to get rid of the pill at some point) but we thought using an English letter H for a global game would've been suboptimal compared to the green +, which is also used as health in Doom Eternal, I believe.

Specifically, I thought it'd be neat if the Hitler change was a reference to what was done in the German version of MachineGames' Wolfenstein games. The logo is the one they use there, and they also removed the mustache. Fitting considering the level was already a Wolf3D reference!

It's the Staatmeister from Wolfenstein 3D for the Super Nintendo. Turns out Doom actually takes place in that universe, not the mainstream Wolf one. Also his modified armband is obviously a prototype of the symbol for the Triad, which connects the whole thing to the deep lore of Rise of the Triad. Which in turn means this all also takes place in the same universe as Heretic and Hexen, and that's why Corvus is in the introduction to one of the Wolf games. I mean, obviously. :)
