What was the biggest BS death you had during a run this year?

For me, saveless 100 percent run of man on the moon.. last fight.. power went out.

What comes to mind is this map by @Kuro_mahoh --- @Biodegradablewas there to laugh his ass off when I survived this crazy fight but died stepping on this tiny pool of acid

Most likely for me was death by a rando homing rev rocket that kept following me and which I managed to completely ignore.

I got stuck on an Imp's fat head while walking over an edge to drop down and get away from a Cyberdemon and then got a rocket where the sun don't shine.

In Stardate 20x6 map05 in the fight with 2 archviles, 2 barons, and 2 hell knights that teleport in I was hit by a high damage roll hk fireball while behind a pillar taking cover from archvile fire. This hit barely pushed me slightly into the vile’s field of fire so I was zapped. This did not kill me however. I was launched by the vile attack just barely into this small crevice at the entrance to the arena. This crevice had damaging floor at the bottom so then I died after about 10 seconds. 
 

A series of coincidences and bad luck all lining up perfectly to kill me.

 

This is of course a single segment attempt at a UVMax because that’s pretty much all I do nowadays other than mapping.
