I second Lilith.pk3. I would put my terrible mapset from 2016 or so out here but I'd actually rather you didn't play it. Black Tower from the Doom 2 Master Levels isn't quite as "surreal" as a lot of these, but it was the first that jumped to mind. A lot of Doom: The Lost Episode by @Xaser is pretty surreal in its aesthetics.
