Did you understand what each difficulty means when you played Ultimate Doom for the first time by just reading the name of the difficulty?

I always found it annoying how Rise of the Triad's skill 1 and 2 names are nearly identical in tone, and so are skill 3 and 4 names. In every variant.

Yes, obviously?  It was the same as with Wolf 3-D: the lower down the list, the higher the difficulty.

Then they added NIGHTMARE at the very bottom! =)

Or are we being pedantic about the actual fun descriptions?

I think I have a good idea what difficulty does what from the start, that was when I was 5 and English isn't my first language.

I have been 8 years old but it's pretty obvious what was intended by the difficulty names. Like you can probably tell that the "NIGHTMARE" font was a clue that difficulty was not actually intended to be played in a remotely serious fashion! The rest just followed the Wolf3D formula of taunting people on the easiest difficulty level while calling you a badass on the higher ones essentially.

Other than Easy -> Hard, no, absolutely nothing meaningful is contained in the names. They might as well be named Skill 1, Skill 2, Skill 3 and you would have the same amount of information.

They all follow a trend of how much do you want the game to beat on you. Hurt Me Plenty is you asking for a lot more punishment than Hey, Not Too Rough.

I knew that Ultra-Violence is Hard, but I was scared that I would see something like "Sunder" or "Chillax" level of difficulty, so I've chosen Hurt Me Plenty for my first walkthrough, lol
