I've put a copy of my template here. Feel free to use it. Where an asterisk appears, this indicates that some information needs to be added in, or one option needs to be deleted (for an iwad demo, you'll need to edit the first line somewhat more extensively). I've put links to this forum and the DSDA site at the bottom.

I would have put this in my own webspace, but I can't access it right now.
