I think it would be silly if you shot an enemy in Doom 4 and they instantly made a pain noise for every bullet that hit them. Well the kinetic impact is pretty much instant, as in when you get shot in the shoulder, the kick of the bullet hitting your shoulder is going to push your shoulder back, turning your upper body in that direction. Reactions like grabbing it in pain is delayed, yes. Screaming is a little more complicated. A shocked or startled yell when you hear the gun go off will be almost instant again, but yelling in pain will be delayed. If you've ever seen someone get shot or video of someone getting shot, if they start screaming and crying it's usually from the panic and realization of I'VE BEEN SHOT! rather than the actual pain. Kind of like playing Goldeneye deathmatch on N64 with rocket launchers only.
